2. Insurance Patients: If, for any reason, your insurance company does not cover your treatment costs, you acknowledge and accept financial responsibility for all treatment costs and will pay any fees due to Dr. Zelina Medina

3. Cancellation Policy: Please inform office at least 24 hours in advance in the event you need to cancel or reschedule, or you will incur a $75 cancellation fee per cancellation. NO EXCEPTIONS.

4. Occasionally, images of a treatment are taken for educational and marketing purposes.

We will always request during treatment before we do so. Your signature authorizes the use of such image(s) by AcuVida.
